Click on the button below for your event, and you can print your form (it is a two-sided document).  Please note that the "Offline Donations Official Rules and Regulations" are contained in the PDF and are the back side of the form. Cash donations are not accepted.

NEW - The deadlines to accept offline (check) donations have been extended through the close of donations for each event. Donations close two weeks after each event, and November 7 for the Challenge.
